Practice Terms & Informed Consent

These Terms of Medical Services (“Medical Services Terms”) govern your use of medical services provided by Suneha Sundaram, MD PLLC, doing business as Triangle Longevity Clinic (“Triangle Longevity Clinic,” “Practice,” “we,” “us,” or “our”), including telehealth services (collectively, the “Medical Services”).

1. Nature of Direct-Pay; Not Health Insurance

Triangle Longevity Clinic is a direct-pay, cash-pay medical practice. We do not participate in, file claims with, or accept payment from commercial health insurance plans, Medicare, or Medicaid. Services are paid directly by the patient.

This membership is not a contract of insurance and does not replace major medical or catastrophic health insurance. Patients are strongly advised to maintain separate coverage for hospitalizations, surgical care, and emergencies.

2. Off-Label and Compounded Therapy Disclosures

As part of individualized longevity medicine, Dr. Sundaram may discuss treatment options involving off-label use of FDA-approved medications or compounded peptide and hormone formulations.

Off-label prescribing is a lawful and common medical practice. However, patients understand that certain peptides or compounded products may not have undergone direct FDA review for specific anti-aging or longevity indications. Full informed consent and potential risks are reviewed with each patient prior to starting therapy.

3. No Guarantee of Medical Outcomes

Medicine and longevity science involve inherent biological variability. Triangle Longevity Clinic does not guarantee any specific biological age score, disease prevention guarantee, or specific biomarker shift. Clinical decisions reflect Dr. Sundaram’s best professional judgment based on current clinical literature and individual diagnostic data.

4. Patient Privacy & HIPAA Compliance

We maintain strict confidentiality policies complying with the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Protected Health Information (PHI) will never be sold or disclosed for marketing without explicit, revocable written authorization.

5. Telehealth Consent

Some consultations and follow-up reviews may be conducted via secure synchronous video or telephone. Patients acknowledge that telehealth cannot substitute for an in-person physical examination when clinically required, and agree to accurately report their physical location within North Carolina during telehealth encounters.

6. Emergency Medical Care Disclaimer

Triangle Longevity Clinic is an outpatient preventative and longevity clinic. We do not provide 24/7 emergency medical coverage. If you believe you are experiencing a life-threatening medical emergency, call 911 or proceed immediately to the nearest hospital emergency room.
