What Is Included: The membership fee covers all ongoing physician consultation, clinical care management, treatment planning, and monitoring provided directly by Dr. Suneha Sundaram and Triangle Longevity Clinic.
What Is Billed Separately: Outside laboratory fees (specialized or repeat outside blood draws beyond baseline), third-party pharmacy medication costs (injectable peptides, compounded hormones), outside imaging facilities (DEXA, CT coronary scoring, MRIs), and specialist referrals will be billed separately at clear, pass-through or negotiated rates communicated in advance.

In many instances, Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA) can be used to reimburse medical consultations, physician evaluations, and laboratory diagnostics. We recommend confirming with your HSA/FSA administrator; we are happy to provide an itemized receipt upon request.
In accordance with Section 13 of our Direct-Pay Membership Agreement, if you cancel prior to the end of your term, we issue a refund for each full calendar month remaining ($583.33/month on the 6-month plan; $500.00/month on the 12-month plan) upon 30 days' written notice. The initial evaluation fee ($1,200) is earned once the initial visit and diagnostics have occurred.
No. Our direct-pay membership covers outpatient longevity medicine and clinical prevention. It is not health insurance and does not cover emergency department visits, hospitalizations, or specialized surgical care. We strongly advise all patients to maintain separate catastrophic or major medical health insurance.
Dr. Sundaram is a licensed physician in North Carolina. In-person or telehealth care requires patients to be physically located within North Carolina at the time of the clinical encounter in compliance with medical licensure regulations.
